To troubleshoot a form that isn't submitting, please take a look at the items below and make sure you are all good:
- Make sure you have a button added to your pop up and the button action when clicked is set to submit form.
- Make sure your form visibility is turned on.
- While there are some exceptions, forms generally require an email address to submit. Make sure the email field is turned on.
